Your name, mailing address, phone number, and email address. A statement (under penalty of perjury) saying you believe the takedown was a mistake or misidentification. A statement that you agree to the jurisdiction of the nearest U.S. federal district court (or any court where we operate if you live outside the U.S.), and that you’ll accept legal service from the person who made the original complaint. Your signature (typed or scanned). Once we receive a valid counter-notice, and unless the original claimant takes legal action, we may restore the content in question after a brief waiting period.
